All HMO's have done in essence is transfer the profit center of the doctors under the old health insurance system to contain the labor cost. They have tied financial incentives for doctors who use alternative less costly treatments. Before HMO's it was easier for people to get health insurance because people who were ill were not treated as a liability on a Corporate P&L.
